Also the patient was placed on antiglaucoma medications. The patient had a history of recurrent corneal ulcerations in both eyes due to severe dry eyes. Examination revealed no light perception with a disfiguring central opaque corneal bulge in the left eye (figure 1). Anterior segment optical coherence tomography (OCT) revealed a marked thickening of the stroma with diffuse round empty spaces between stromal fibres corresponding to intrastromal silicone oil droplets (figure 2). No treatment was offered because of poor prognosis. Silicone oil is used frequently as adjunct to modern vitreoretinal surgery because of transparency, minimal retinal toxicity and good intraocular tamponade. Complications include cataract formation, glaucoma and oil keratopathy. Silicone oil keratopathy manifests as corneal oedema, bullous keratopathy and rarely white cornea. The corneal endothelial damage stems from glaucoma and silicone oil touch of the endothelium. When the endothelium fails, silicone oil can penetrate into the stroma. Few histopathogical studies have been reported in eyes undergoing penetrating keratoplasty for severe silicone oil keratopathy. Ardjomand et al demonstrated in one corneal button that silicone oil traverses the cornea through defects in the Descemet membrane. Sekundo et al noted the presence of a posterior collagenous layer above a severely attenuated endothelium in eight corneal buttons. OCT allows in vivo examination of the stroma and showed diffuse silicone oil droplets in between stromal fibres. This whitish colour of the cornea is similar to the colour of emulsified silicone oil.

Operative and postoperative complications

A retrospective study of the effects of silicone oil removal was carried out on 85 patients who had undergone pars plana vitrectomy and silicone oil exchange for giant retinal tears or proliferative vitreoretinopathy. Silicone oil was removed either as part of the treatment of anterior segment complications such as glaucoma and keratopathy (25 patients) or in order to prevent these complication...

Anterior Segment Optical Coherence Tomography Imaging of Central Toxic Keratopathy.

The authors report anterior segment optical coherence tomography (OCT) imaging findings in a case of central toxic keratopathy following laser in situ keratomileusis (LASIK) surgery for low hyperopia. OCT imaging 1 month after surgery demonstrates that the flap thickness is maintained and the location of stromal tissue loss is just posterior to the flap in the stromal bed in the affected area. ...

Three-Year Efficacy and Safety of a Silicone Oil-Filled Foldable-Capsular-Vitreous-Body in Three Cases of Severe Retinal Detachment

PURPOSE We previously designed a novel foldable capsular vitreous body (FCVB) to treat severe retinal detachment and evaluated its performance in a 1-year follow up study. The purpose of this study was to determine the efficacy and safety of a silicone oil (SO)-filled FCVB in a 3-year follow-up. METHODS Standard three-port pars plana vitrectomy was performed, and the FCVB was triple folded an...

Basal iridectomy at 6 o'clock in the aphakic eye treated with silicone oil: prevention of keratopathy and secondary glaucoma.

In the aphakic eye, with intact iris diaphragm, silicone oil has frequently caused a pupillary block. In this situation aqueous humour accumulates behind the iris and forces silicone oil through the pupil into the anterior chamber. An iridectomy at the 6 o'clock position can effectively prevent this pupillary block. The iridectomy allows free passage of aqueous to the anterior chamber which rem...

Anterior-segment optical coherence tomography investigation of corneal deturgescence and epithelial remodeling after DSAEK.

PURPOSE The aim of this study was to evaluate via Fourier-domain anterior-segment optical coherence tomography 3-dimensional corneal, epithelial, and graft thickness changes after Descemet stripping automated endothelial keratoplasty (DSAEK). METHODS Sixteen eyes were investigated preoperatively and up to 6 months postoperatively for preoperative and postoperative central corneal thickness (C...
